Carpal tunnel relief without surgery

Splints, hand therapy and daily changes that ease numbness and wrist pain

The short answer

Carpal tunnel relief without surgery is possible for most people with mild to moderate symptoms. The first steps are a wrist splint worn at night to keep the wrist straight, fewer hours of hard gripping and bent-wrist positions, and gentle nerve and tendon gliding exercises. Hand therapy adds hands-on care, a custom splint and a plan for work and home tasks. If numbness keeps waking you after several weeks, a guided steroid injection can calm the swelling around the nerve. Constant numbness, weakness or a wasting thumb muscle needs prompt evaluation, because the nerve may need surgery to recover.

Carpal tunnel syndrome happens when the median nerve is squeezed in the narrow tunnel at the front of the wrist. It causes numbness, tingling and burning in the thumb, index and middle fingers, often at night, and it can make you drop things. Many people assume surgery is the only answer, but carpal tunnel relief usually starts with simple changes that take pressure off the nerve. The videos show hand therapy sessions for carpal tunnel.

Carpal tunnel relief at home

  • Night splint: a wrist splint that keeps the wrist straight while you sleep, when most symptoms flare
  • Change the grip: use a lighter grip on tools, phones and steering wheels, and take a break every 30 to 45 minutes from repetitive hand work
  • Set up the desk: keyboard and mouse at elbow height with the wrists level, not bent up or resting on a hard edge
  • Nerve and tendon gliding: gentle exercises that help the nerve slide through the tunnel; a therapist can show the right ones for you
  • Shake it out: shaking the hands or changing position when tingling starts often eases it

Hand therapy for carpal tunnel

When home steps are not enough, occupational and hand therapy gives a structured plan: a custom-fitted splint, hands-on work for the wrist and forearm, nerve gliding, strengthening once symptoms settle (see grip strength exercises), and changes to the tasks that trigger symptoms at work and at home. The first two videos show this kind of session, including grip work with a therapy ball and wrist exercises on foam blocks. Hand therapy is offered at our Brooklyn office only, where Valerie Khodorkovskiy, OTR provides occupational therapy.

When to consider an injection or surgery

If numbness still wakes you after several weeks of splinting and therapy, a physician may suggest a steroid injection into the carpal tunnel, placed with ultrasound guidance. It often eases symptoms for weeks to months and is described on our carpal tunnel treatment page. A surgical opinion makes sense when numbness is constant, the hand is weak, the muscle at the base of the thumb is shrinking, or nerve testing shows severe pressure on the nerve.

Make sure it is carpal tunnel

Tingling in the hand can also come from the elbow or from a pinched nerve in the neck, and the treatment is different for each. An EMG/NCV test confirms carpal tunnel and shows how severe it is. Our guide to numbness and tingling in hands covers the other causes, and chronic wrist pain treatment covers wrist pain that is not from the nerve.

Questions patients ask

Can carpal tunnel be relieved without surgery?

Yes, for most people with mild to moderate symptoms. A night splint, changes to grip and desk setup, nerve gliding exercises and hand therapy often ease symptoms, and a guided injection can help if they persist.

How long does it take for carpal tunnel to get better without surgery?

Many people notice less night numbness within two to four weeks of wearing a splint and changing their habits. A full course of hand therapy often runs six to eight weeks.

Does wearing a wrist splint at night help carpal tunnel?

Yes. Keeping the wrist straight at night lowers the pressure on the median nerve, and it is one of the most effective first steps for carpal tunnel relief.

What does hand therapy do for carpal tunnel?

An occupational or hand therapist fits a splint, does hands-on work for the wrist and forearm, teaches nerve gliding and strengthening, and changes the tasks that trigger symptoms.

When does carpal tunnel need surgery?

A surgical opinion makes sense for constant numbness, weakness, a shrinking muscle at the base of the thumb, or severe nerve pressure on an EMG/NCV test.
