Precision Care Medical PC, doing business as Precision Care Medical & Sports Performance and Precision Care Spine & Pain Center (“Precision Care,” “we,” “us,” or “our”), operates the website https://precisioncaremed.com (the “Website”). This Website Privacy Policy explains what information the Website collects, how we use it, who we share it with, and the choices you have.
This policy covers only the Website. It does not cover your medical records or other health information we keep as your health care provider. That information is protected by federal and New York law and is described in our separate Notice of Privacy Practices, available at https://precisioncaremed.com/notice-of-privacy-practices/ and at our offices.
Summary
- We collect the information you type into our appointment request form (your name, phone number, email address, and preferred appointment date) so we can contact you to schedule.
- Please do not include medical details, symptoms, diagnoses, insurance numbers, or other health information in the form. We will collect what we need privately when we contact you.
- We count visits with Koko Analytics, a tool that runs on our own Website server, uses no tracking cookies, and does not send data to other companies.
- We use Google Analytics only if you click “Accept” in our cookie banner. Google Analytics is never loaded on our appointment request page or thank-you page.
- We do not use Meta/Facebook advertising pixels.
- Some pages show content from Google (maps, fonts, reviews) and YouTube (videos).
- We do not sell your personal information.
Information you give us
Appointment request form
When you use our appointment request form, we collect your name, phone number, email address, and preferred appointment date. The form is sent to our own Website server, which is hosted by WP Engine, and delivered to our staff so we can contact you. An online request is not a confirmed booking until our office contacts you.
Please do not include medical details in the form. Do not enter symptoms, diagnoses, injuries, medications, insurance or Social Security numbers, or other health information. If you need to share health information, we will collect it from you privately when we speak with you or at your visit.
The form is not for emergencies. If you have a medical emergency, call 911 or go to the nearest emergency room.
Emails and phone calls
If you email us (for example, at info@precisioncaremed.com) or call us, we receive the information you choose to share. Regular email is not fully secure, so please avoid sending sensitive health information by email.
Information collected automatically
Server and security logs
Like most websites, our hosting provider, WP Engine, automatically records technical information when a browser requests a page. This can include your IP address, browser type, the page requested, the referring page, and the date and time. This information is used to deliver the Website, keep it secure, and fix problems.
Koko Analytics (on our own server)
We use Koko Analytics, a WordPress plugin, to count visits, the pages viewed, and the websites that referred visitors to us. This data is stored only in our own Website database on our server, hosted by WP Engine. Koko Analytics does not send data to any other company and does not use cookies. It keeps only totals, such as how many times each page was viewed. It does not store your IP address, does not build a profile of you, and does not record anything you type into our forms. Because it does not use cookies, it runs without your choice in the cookie banner.
Google Analytics 4 (only if you accept)
If you click “Accept” in our cookie banner, we use Google Analytics 4, a service from Google LLC, to understand how visitors use the Website. Google Analytics does not load, and nothing is sent to Google Analytics, unless you accept. If you accept, Google Analytics collects information such as the pages you view, the page addresses (URLs), how you arrived at the Website, your general location based on your IP address, your device type, operating system, and browser, and how long you stay. It uses cookies such as _ga to tell visits apart.
Google Analytics is never loaded on our appointment request page or our appointment thank-you page, so Google does not receive information about appointment requests or anything you type into the form.
No advertising pixels
We do not use Meta/Facebook advertising pixels or similar advertising tracking tools on the Website.
Content from other companies on our pages
Some pages show content that is loaded directly from other companies’ servers. When your browser loads it, that company can receive your IP address and browser information and may set or read its own cookies, under its own privacy policy.
- Google Maps maps on our location pages. Maps load content from Google.
- YouTube video thumbnails and videos (YouTube is owned by Google). Videos load content from YouTube.
- Google Fonts, which display the lettering on the Website.
- A Google Reviews widget, which shows reviews of our practice posted on Google.
- Images delivered through WordPress.com, an image service run by Automattic Inc. that delivers some of our Website images faster.
How we use information
- To respond to your appointment request and schedule your visit.
- To answer your questions and communicate with you.
- To operate, maintain, secure, and improve the Website.
- To understand, in summary form, how visitors use the Website and how many appointment requests it receives.
- To comply with the law and protect our rights, our patients, and the public.
We do not use Website browsing information to identify a visitor’s health conditions, we do not use it for advertising, and we do not use it to make decisions about anyone’s medical care.
Who we share information with
We share Website information only as described in this policy:
- Service providers that run the Website for us, listed below.
- Legal and safety reasons, when the law requires it or when needed to protect the rights, property, or safety of our practice, patients, or others.
- A business change, such as a merger or sale of the practice, subject to applicable law.
- WP Engine (what it does for the website: Hosts the Website, its Koko Analytics data, and appointment form submissions on our behalf; privacy information: https://wpengine.com/legal/privacy/)
- Google LLC (what it does for the website: Google Analytics 4 (only after you accept), Google Maps, YouTube, Google Fonts, Google Reviews content; privacy information: https://policies.google.com/privacy)
- Automattic Inc. (what it does for the website: Delivers some Website images (WordPress.com image service); privacy information: https://automattic.com/privacy/)
We do not sell your personal information
We do not sell personal information collected through the Website, and we do not share it with advertisers.
Cookies and your choices
Cookies are small files that a website stores in your browser. Some are needed for the Website to work. Others are used for analytics.
Our cookie banner
When you first visit the Website, a cookie banner (provided by the Complianz plugin) asks whether you accept analytics cookies. Google Analytics loads only if you click “Accept.” If you do not accept, nothing is sent to Google Analytics, and the Website still works. You can change your choice or withdraw your consent at any time using the “Cookie settings” link in the footer of every page. Withdrawing consent stops future collection; it does not undo collection that happened before. The banner stores your choice in a cookie so it does not ask you on every page.
- Cookies beginning with cmplz_ (set by: Our cookie banner (Complianz); purpose: Remembers your cookie choice; typical duration: Up to about 1 year)
- _ga and _ga_ followed by an ID (set by: Google Analytics (only after you accept); purpose: Tells visits and visitors apart for statistics; typical duration: Up to 2 years)
Koko Analytics does not set cookies. Google Maps, YouTube, and the reviews widget may set or read their own cookies when they load.
Other ways to control tracking
- Browser settings: most browsers let you block or delete cookies and block third-party cookies. Blocking all cookies may affect how some sites work.
- Google Analytics: you can install Google’s opt-out browser add-on at https://tools.google.com/dlpage/gaoptout. To learn how Google uses information from websites that use its services, see https://policies.google.com/technologies/partner-sites.
Our Website does not currently respond to browser “Do Not Track” signals, because there is no common standard for them. Your choice in our cookie banner controls Google Analytics on the Website.
How long we keep information
- Appointment requests: we keep form submissions as long as needed to respond to your request and as required by law. If you become a patient, information that becomes part of your medical record is kept as required by law and described in our Notice of Privacy Practices.
- Google Analytics: user-level analytics data is kept for 14 months, then deleted by Google. Summary reports that do not identify individuals may be kept longer.
- Koko Analytics: summary visit statistics are kept on our own server and do not include your IP address.
- Other providers keep information according to their own policies.
How we protect information
We use reasonable administrative, technical, and physical safeguards to protect information collected through the Website, including an encrypted (HTTPS) connection for the Website and its forms. No website or internet transmission is completely secure, so we cannot guarantee absolute security. If we learn of a security incident affecting your information, we will notify you as required by law.
Children’s privacy
The Website is not directed to children under 13, and we do not knowingly collect personal information online from children under 13. If you are a parent or guardian and believe your child has given us information through the Website, contact us and we will delete it. If you are requesting an appointment for a patient under 18, a parent or legal guardian should submit the request.
How this policy relates to HIPAA
As a health care provider, we follow the federal HIPAA Privacy Rule and New York law for protected health information, such as your medical records, treatment, and billing information. How we use and share that information, and your rights, are explained in our separate Notice of Privacy Practices at https://precisioncaremed.com/notice-of-privacy-practices/.
This Website Privacy Policy covers general information collected when people visit our Website. We do not use Website browsing information to identify patients or their health conditions, and we do not combine it with medical records. Google Analytics is not loaded on our appointment request or thank-you pages, and no analytics tool receives what you type into our forms. Once you become a patient, the information you give us for your care is handled under our Notice of Privacy Practices.
Your privacy rights, including for New York residents
We respect the privacy rights of all Website visitors, including New York residents. Depending on applicable law, you may ask us to:
- tell you what personal information we have collected about you through the Website;
- correct information that is inaccurate;
- delete information you submitted through the Website, unless we must keep it for legal reasons or because it is part of your medical record; and
- stop using your information for analytics (you can also do this at any time with the “Cookie settings” link).
To make a request, contact us using the information below. We may need to confirm your identity before we act on a request. We will respond within a reasonable time and as required by law, and we will not treat you differently for making a request. If we experience a data breach involving your personal information, we will notify you as required by New York and federal law. Rights regarding your medical records are described in our Notice of Privacy Practices.
Links to other websites
The Website may link to websites we do not control, such as Google Maps, review sites, insurance companies, or social media pages. Their privacy practices are governed by their own policies.
Changes to this policy
We may update this policy when our Website or practices change. We will post the updated policy on this page and change the “Last updated” date at the top. If we make significant changes, we will post a notice on the Website.
